today we pulled apart a spare LAD regulator to try to answer your queries

Q1 - the spare LAD had the small cap screwed down all the way -- no guarantee that it works for you, but the cap rotates very easily so it seems like it would unscrew itself if not down tight.

Q2 - under the large screw cap we found a 3.5 mm ball,

under the small cap, a 4.5 mm ball

Did not pull the whole LAD apart and so did not find the third ball

Q3 - first, the rotating valve shown in your photos differs from the spare we had, the shaft of ours has a square peg on the end

however, with the large cap off and all parts out, we put a blunt probe down to rest against the rotating valve

turn the valve anti-clockwise and find the point at which the probe starts to fall down. Without moving the shaft, install the lever so that the bolt hole in the lever lines up with the hole on the body.

another unit

tighten up on the pinch bolt and reinstall the internals

Q4 can't hurt to clean the body

Q5 - the spare unit spun very easily - no other info available at this time.

Hope this helps
